Removed from a decommissoned project. As far as I can tell this device is acting as a multi channel amplifier circuit for accelerometers that were vibration monitoring fan vibrations. With the output going to an analog input card on a PLC. 

As this device has no data sheets or information I cannot provide any further support or information on this product.
